We ask that our patients complete the appropriate forms prior to their appointment. By doing this we will be able to provide our patients with a better service.

• Established patients Release of Medical Records (The practice may charge 45 cents per page for copied records or bills.)

All patients should bring their insurance cards and information about the primary insured person on the health insurance policy. Copays are due at the time of service.

Payments are expected when services are performed unless other arrangements are made in advance.

The practice may charge up to forty-five cents per page for copied records or bills.
